We excited to be working as a delivery partner for Support Staffordshire, on providing community solutions to hospital discharge for pathway 1 and pathway 0 patients.
According to The Health Foundation UK, most people admitted to hospital want to get better and get home, as soon as possible. Long stays in hospital can lead to loss of muscle strength – by up to 5–10% within the first 7 days – which can hinder their return to living independently, or increase the level of support they need.
Analysis by Age UK shows that the average excess bed day – the cost of a patient remaining in a hospital bed after their treatment has ended – costs £346
Our role is to work alongside the Transfer of Care Support Staffordshire Team to provide personalised care solutions that make a smooth transition from hospital to home (Community Together CIC cover Tamworth, Lichfield and Burntwood areas) and longerterm, supporting people in independent living in their communities. Some of the things we do to work together to support this are:
Fitting Key Safes
Food Shopping
Organising House Cleans or House Clearances
Furniture Removal and Moving Furniture
Welfare Check In Calls
